By August 3, officials confirmed 3,874 cases and 1,751 deaths, marking Congo’s most extensive documented Ebola outbreak and the second largest worldwide. The country reached 1,000 cases just 40 days after initiating its response, whereas a major outbreak starting in 2018 took approximately 235 days to surpass that number.

Health authorities declared the outbreak on May 15 following laboratory detection of Bundibugyo virus in Ituri province. Investigations later uncovered evidence that infections had begun months earlier near Mongbwalu. Many early patients exhibited symptoms similar to malaria and other common illnesses. Initial lab tests primarily focused on the better-known Zaire Ebola species, delaying recognition of the true threat and allowing the virus additional time to spread through households, clinics, mining communities, and trading areas before testing and isolation efforts expanded.
The presence of the Bundibugyo strain has also limited available medical tools for responders. Vaccines and antibody treatments approved for Ebola target Zaire ebolavirus, responsible for Congo’s 2018 to 2020 outbreak. Currently, there are no licensed vaccines or proven specific treatments for Bundibugyo virus disease. Healthcare workers rely on rapid diagnostics, isolation, supportive care, infection control, and safe burials. Delayed detection caused contact tracing delays
The outbreak has extended beyond Mongbwalu into numerous health zones across eastern and northeastern Congo, with Ituri remaining the epicenter. Cases have also been reported in North Kivu, South Kivu, Haut-Uele, and Tshopo. By July 30, response teams had tracked 17,863 contacts, though follow-up efforts have been inconsistent, especially in regions affected by insecurity and difficult terrain. Many new patients have been identified outside existing contact lists, indicating surveillance teams have not yet fully mapped every transmission chain.
Conflict has hampered case detection and patient management, with armed attacks blocking roads, disrupting health activities, and causing teams to halt field operations. The high mobility of populations between mining sites, markets, towns, and displacement camps complicates daily contact monitoring. Healthcare facilities face shortages of protective gear, trained personnel, transportation, and laboratory services. As of July 30, Congo reported 151 infections and 44 deaths among health workers, adding pressure to an already overwhelmed response effort.
Vaccination gaps and insecurity speed up spread
Ebola transmits primarily through direct contact with the blood or body fluids of an infected individual, especially in homes, clinics, and burial settings lacking adequate infection control. Over 60% of recent fatalities occurred outside treatment centers, complicating safe burial procedures and contact investigations. To combat this, Congo’s health ministry, the World Health Organization, and Africa CDC have expanded laboratory capacity, established more treatment sites, increased border screenings, and intensified public awareness efforts. Yet, the response has still struggled to keep pace with the rapid and widespread nature of new infections.
Uganda declared its linked outbreak over on July 28 after 42 days with no local transmission, and France also reported no secondary cases from its single treated patient. Nevertheless, Congo continues to be the epicenter of ongoing transmission, with a death rate near 45% as of early August. The epidemic is accelerating due to late detection, the absence of strain-specific vaccines and treatments, missed contacts, ongoing conflict, staffing shortages, and population mobility—all factors that have expanded transmission pathways. These conditions set the current Bundibugyo outbreak apart from earlier Ebola episodes in Congo, emphasizing critical response challenges.
